Description
Upon completing your booking you will receive a confirmation email and booking form that needs to be filled out and signed. Cancelation Policy: To ensure that we can provide the best service to all of our patients, we ask for a minimum of 24 hours' notice for appointment cancellations or rescheduling. No shows will be charged the full price of the appointment. To book an appointment a credit card must be entered in to the booking system even if we will be direct billing to your insurer.
